Please use the Request for Medical Records Release form by clicking the button below.


  1. Print, Complete, and Sign the form
  2. Deliver the form to our office via Email, Postal Service, or Drop-off at our office
  3. If via Email, after printing, completing, and signing the form, scan and email to: kdeloach@waccamawurology.com
  4. If using a mobile phone, after printing, completing, and signing the form, take a clear picture of the form, then email it as directed in #3 above.
  5. If dropping off at the office, please note the hours through the end of April 2025.
